If you're making you need a Form 1. Just Google ATF Form 1 and you'll get this link http://www.atf.gov/forms/download/atf-f-5320-1.pdf

Same thing for a Form 4 transfer.

No, a form 4 is for a transfer. Make sure you read through the stickies and a few other posts on how to fill out a form 1.

It sounds like you want a form 1 and you have to pick a specific caliber for the application as they won't accept "Multi" in block 4c. In block 4h you can put any additional markings that you deem fit but I just listed my trust name and location.
